Apple Football School cannot accept liability or be held responsible for; · Any injury caused whilst travelling to or from any course · Any injury caused during a contact sport such as Football · Any illness as a result of or during the course · Any loss or damage to personal belongings whilst in attendance, or travelling to the course
* All Medical information / details must be fully disclosed * Only bookings in advance can be accepted for safety reasons * Pupils will be registered each day – if unable to attend for any point of the course please let the coach in charge know ASAP. * Pupils will only be allowed to leave with the designated person (application) * Parents are responsible for providing a packed lunch and snacks, drinks, suitable equipment / clothing and putting on Sun Screen before sessions (as relevant to the weather). Please note: any additional Sun Screen which is required during the day will have to be applied by the pupil * Shin Pads MUST be worn - Apple Football School has the right to refuse participation in activities without Shin Pads * Parental consent is given to Apple Football School to administer Emergency Aid treatment / First Aid if necessary, if a player becomes ill or injured. Consent is also given to Apple Football School to take a child to hospital if necessary. If such incidences occur, contact with parents will be attempted. * Apple Football School has indoor changing areas / lunch areas available * Poor Weather -again changing areas can be made available- suitable activities will be organised whilst under the care of Apple Football School in the instance of inclement weather * Written notice must be given if your child is unable to attend a course after a booking has been made. Written notice MUST be received at least 7 days prior to any course start date. A £10 admin fee will be charged for cancellation of booking and refund costs. Apple Football School can not offer any refunds for bookings cancelled less than 7 days before any course start date * A full refund will be given (no written notice required) if for any reason Apple Football School has to cancel a course. * Customers use Apple Football School at their own risk. The service is provided without warranty of any kind. This disclaimer of liability will apply to any injury and or damage to pupils caused by poor performance, conduct, negligence, offensive or illegal conduct from pupils, other customers or third parties whether or not participating on the course. Risk of injury lies entirely with the customer. * Bookings only accepted with confirmation of acceptance of Terms and Conditions * To enable courses to run, we require a minimum of 10 pupils– maximum 32 for Football coaching and a minimum of 5 pupils – maximum 16 for Goalkeeping courses
Code of Conduct * Appropriate clothing should be worn (as per weather) * Correct equipment should be worn- boots/trainers , shin pads etc * Pupils should arrive at least ten minutes before coaching starts * Pupils are expected to show positive attitudes and follow all instructions * Pupils will be expected to act safely and responsively * Pupils must be courteous and polite towards team mates, coaches and referees * No swearing, spitting, name calling or racist remarks * Pupils must show respect for equipment and facilities they use * At the end of any matches players must shake hands with players and officials * Let players play, coaches coach, managers manage and referees ref * Parents / Guardians will be expected to collect pupils on time (sign pupils in and out) / inform coach if running late or unable to attend / if there is a change of collector * Parents/Guardians can watch but please be aware this may make your child feel uncomfortable!
